Supported Living Services
Bosom SLS is a non-medical agency with the primary purpose of enhancing the quality of life for individuals with developmental challenges by offering compassionate, personalized, and dignified services as needed, that enable them to remain in their homes or familiar rented environments with desired comfort and independence. We work in liaison, support and with guidelines from Alta California Regional Center (ACRC). We are located in Northern California.
Our vision is to continue being the agency of choice in California for both the individuals challenged by developmental disabilities and their families through our exceptional services.
Our Mission is to provide integrated supported living services enabling the developmentally challenged persons live successfully in homes of their choice that they own or rent, in communities of their choice, exercise maximum control over their lives, develop and enjoy positive sustainable relationships in safe and healthy spaces.
The focus of these services is for the individuals to live independent and productive lives and in their communities. It is Bosom’s desire to see each individual who is entrusted to us thrive, prosper, reach highest potential to the best of their abilities and if possible, get to extra-miles of enrichment via personal efforts, innovation and via supports in and around their lives.
Individuals are encouraged to make choices, exist wholly and well supported by the circle of supports that comprise a network of friends, family, associates, professionals, natural supports, Bosom staff and the community as needed. Bosom’s mission responds promptly and in a dignified manner, ensuring individuals feel comfortable and valued in their own homes or/and familiar surroundings. Bosom is driven by a deep commitment to enhancing the well-being and quality of life of those we serve. The passion behind Bosom’s work lies in the belief that every individual deserves to receive care and support in a dignified, personalized and respectful manner, regardless of their circumstances. By responding promptly with attentive care and active listening the agency aims to not only assist the individual to meet their immediate needs and wants but also to facilitate meaningful connections, trust, and a sense of belonging within their communities.

- Provide responsive and personalized non-medical services tailored to the unique needs of each individual with reference to Individual Program Plan (IPP) and keenly implement the Individual Service Plan (ISP).
- At all times maintain high standards of quality and compassion in all interactions and service delivery.
- Foster a supportive and nurturing environment that promotes dignity, respect, and well-being in sustainable approaches.
- Continuously improve and adapt our services to meet the evolving needs of our clients and their families.
